Tadalafil (Cialis)
Tadalafil is an oral medication primarily used to treat erectile dysfunction (ED) and benign prostatic hyperplasia (BPH). It belongs to a class of drugs called phosphodiesterase type 5 (PDE5) inhibitors, which also includes sildenafil (Viagra) and vardenafil (Levitra).

For erectile dysfunction, tadalafil works by enhancing blood flow to the penis. Sexual stimulation leads to the release of nitric oxide, which then stimulates the enzyme guanylate cyclase. This enzyme increases levels of cyclic guanosine monophosphate (cGMP), which causes the blood vessels in the penis to expand, allowing more blood to flow in. As the blood vessels contract after the cGMP is broken down by PDE5, the erection ends. Tadalafil works by blocking PDE5, allowing cGMP to persist longer, leading to prolonged blood flow and erection.

Though the exact mechanism for treating BPH is not well understood, tadalafil may help by relaxing muscles in the prostate and bladder. This could reduce urinary difficulties, such as lower pressure on the urethra and easier urine flow.

Warning: Tadalafil should not be taken with nitrate medications for chest pain or heart problems, as this can cause a dangerous drop in blood pressure. Nitrates are also found in certain recreational drugs (e.g., “poppers”).

Tadalafil was approved by the FDA in 2003.
